Do I need a referral?
Patients 19 and under, 65 and over, diabetics and patients with eye disease require a referral for their initial appointment. If you fit into the above categories and have not been seen in our office for over 2 years, you require a new referral to be seen by Dr. Goldberg again. You do not need a referral to see Dr. Anna Yu
How long will my appointment take?
You should plan to be in the office for at least 2 hours. If you are coming by wheeltrans please book 3 hours.
What should I bring to my appointment?

Your Ontario Health Insurance Card (OHIP card)

List of current medications and drug allergies

The prescription glasses that you use

Sunglasses / baseball cap as you may get dilating drops that make your eyes sensitive to light

Contact lens wearers can wear their lenses to the office but please bring your glasses and a contact lens case containing storage solution

The name and phone number of your family doctor
Do I have to pay for anything?

Age 0 – 19: OHIP will cover your routine eye exam

Age 20 – 64: OHIP does not cover routine eye exam except under specific circumstances, ie you are diabetic or have certain specific eye problems

Age 65 and up: OHIP will cover your routine eye exam

Special tests: Some tests that are helpful in monitoring glaucoma issues are not covered. Payment can be made by credit card (VISA or Mastercard), debit card or cash. We provide you with a receipt to submit to your insurance company.

Uninsured services: pharmacy calls for prescription renewals and some other services are not covered by OHIP and are billed either individually or as part of an annual service fee
Can I drive after my appointment?
There are no driving restrictions if your eyes are dilated. However, your vision will be blurry. You may wish to have a driver. We advise not driving after a laser appointment or after having a lid procedure unless you have cleared this with your doctor first.

By subway: We are conveniently located near the Davisville subway station if you prefer public transportation.

By car: Metered parking is available on Merton Street and Yonge Street. There is also a parking lot on the north side of Merton Street.
